Background
Accurate early prediction of response to neoadjuvant chemoradiotherapy (nCRT) could help tailor treatment in locally advanced rectal cancer, sparing non-responders ineffective therapy and identifying candidates for organ preservation. Radiomics offers a non-invasive way to extract quantitative features from routine imaging.
Methods
We retrospectively analysed 210 patients who underwent nCRT between 2018 and 2024. Radiomic features were extracted from pre-treatment T2-weighted MRI, and a machine-learning model was trained on 70% of the cohort and validated on the remaining 30%. Performance was assessed by area under the receiver operating characteristic curve (AUC).
Results
Pathological complete response was achieved in 38 patients (18.1%). The radiomics model predicted response with an AUC of 0.86 (95% CI 0.79–0.92) in the validation set, outperforming a clinical model based on tumour stage and size alone (AUC 0.68, p < 0.01).
Conclusion
A pre-treatment MRI radiomics model accurately predicted response to chemoradiotherapy and may support personalised treatment decisions. Prospective multicentre validation is required before clinical adoption.
